This includes any request to send tax returns and/or W-2’s to any lender or financial institution. Please complete the highlighted section. Both spouses must sign for a Joint tax return. Individual
The 2009 Tax Organizer can assist you in collecting and reporting information necessary for us to properly prepare your 2009 income tax return. Please complete the organizer sections as appropriate and provide supporting documentation where necessary. Contact Renee Franke at (480) 905-3640 regarding retainer amount due with the individual organizer.
The organizer is fairly lengthy to cover most all possible income and deductions. We have the organizer set-up so you can choose only the forms that apply to you. The first section contains the mandatory forms and questions. Choose only the forms in the second section that apply to you.

2009 Year-End Reporting of Taxable Fringe Benefits
When a company makes one of its vehicles available to an owner or employee without charge, the company must calculate the value of any non-business use of the vehicle and report it on that individual’s Form W-2 as a taxable fringe benefit.
Please prepare one form for each owner or employee who is using a company vehicle (even if two employees share one vehicle), and fax or mail them to our office.
If your company is a Subchapter S corporation, or a Limited Liability Company that has elected to be taxed as a Subchapter S corporation, then the amount of any accident, health or long-term care insurance premiums paid by the company for the benefit of an individual who owns more than two percent (2%) of the company is also considered by the IRS to be a taxable fringe benefit.
Complete the form and return to us so we can provide you with the specific numbers and instructions for including this fringe benefit as taxable wages and the corresponding reporting of this benefit on Form W-2.
Complete and return this form to us if you wish for us to prepare your forms 1099 for 2009. You are required to file all 1099 forms by January 31, 2010. You can email the completed form to Jamie Buterbaugh at Jamie@buterbaughcpa.com

Complete this form so that your employer can withhold the correct federal income tax from your pay. You should consider completing a new Form W-4 each year and when your personal or financial situation changes
Complete this form and return to your employer to elect an Arizona withholding percentage. Arizona withholding is a percentage of the amount of federal withholding withheld. You should consider completing a new Form A-4 each year and when your personal or financial situation changes.
